Observation

110-300-0165(2)(d) - A roll of plastic bags was found in the bathroom within easy reach of children. The provider promptly removed the bags and stored them in a locked drawer. 110-300-0165(3)(c) - A broken wooden section on the play structure near the monkey bars was identified. 110-300-0165(3)(g) - Loose bolt s were noted on the lower portion of the slide, and there was also a loose and rusty monkey bar observed.
